Co-pays are due at the time of service, with no exceptions. We accept Visa, MasterCard, American Express and Discover Cards, as well as personal checks. There will be a $35 charge for returned checks, and checks may not be accepted for future visits.

A physical examination by your family physician in the previous year is strongly recommended.

Please arrive twenty minutes early for your first appointment. This will allow time to complete forms, ask questions and make financial arrangements with the business office before your appointment. Be sure to bring your insurance card on the first visit and notify our office of any change in coverage. We file insurance with most major companies, however your policy may require a referral from your primary care physician or authorization for mental health visits. Please consult your plan booklet and call the 800 number on your insurance card for clarification of plan requirements and provider participation.

Insurance does not pay for appointments not kept or cancelled at the last minute. Please consider our lost time if you “no show” or cancel at the last minute. We request 24 hour advance cancellation notice.

Calls for medication refills will be answered the next business day when your chart is available. Check your medications regularly and plan to see your doctor before the supply is exhausted to discuss changes and new prescriptions.

Our phone is answered 24 hours a day, seven days a week to take urgent calls and cancellations.
